Course Waivers

Core course waiver examinations are held annually at the beginning of each academic year.

Waivers are highly recommended for students with working knowledge of certain core areas in order to avoid frustration in courses that cover known material.

Planning Forms

All Evans School planning forms are available in Excel or Word document formats.

If you are a Master of Public Administration (MPA) student, you must use a plan of study form (21 KB Doc) in addition to your specific program’s planning form.

*Please Note: We will not offer Leadership III (PbAf 545) during the 2007-08 or 2008-09 academic years. In its place, we require that Midcareer MPA students take another elective course focused on leadership or management. Internship Forms

Evans School internship forms, except for the internship waiver, are online forms. For the internship waiver you will need Adobe Acrobat Reader to view it.

• Agency/Student Contract Form
  • Describes the scope of work, your professional objectives, and the agency’s expectations
  • Must be submitted within two weeks of starting your internship
• Internship Evaluation Form
  • Describes the quality of the internship and what you got out of it
  • Must be submitted within two weeks of completing the internship
• Intern Supervisor Evaluation Form
  • Remind your supervisor to complete and submit the form
  • Must be submitted to Career Services during your last week of work

Degree Project Forms

Evans School degree project forms are available in PDF format and require Adobe Acrobat Reader to view.

• Degree Project Proposal Form (34 KB PDF): Must be submitted to the Student Services office before getting a registration code

• Degree Project Sample Title Page (5 KB PDF): Sample format for the title page of your degree project

• Degree Project Summary (12 KB PDF): Must be submitted with your degree project

All International Development Policy and Management Certificate Program forms are available in Word document format and intended for the specified cohort.

• Change of Address Form (25KB PDF): You can either submit this form or contact evansuw@u.washington.edu directly to change your address at the Evans School. The UW Registration Office will need to be contacted separately at 206.543.3868 to change your address in the main university system.
